Our online form lets you tell us the essentials before you call. You can provide your name and contact details, choose your preferred contact method of phone or email, suggest a preferred date, indicate whether mornings or afternoons suit you better, and select the procedure you are interested in — general dentistry, dental implants, dentures and partials, Teeth in Just One Day, IV sedation, a cleaning or check-up, fillers and Botox, or an emergency.
A member of our staff will contact you to arrange the appointment. Adding a short message about your situation helps us allocate the right amount of time for your visit.
What to Bring to Your First Visit

- Relevant medical history, including any conditions affecting treatment or sedation
- Your dental insurance details, so our staff can review your benefits
- Any recent X-rays or records from a previous dental office, where available
If you are considering IV sedation, arrange for a family member or friend to drive you home afterward.

Our three core service areas are Springfield, Champaign and Mahomet. The office sits at 687 E Linton Ave in Springfield, with Champaign and Mahomet patients reaching us directly along I-72.
We also serve Chatham, Decatur, Effingham, Lincoln, Peoria, Quincy, Jacksonville, Bloomington, Taylorville, Edwardsville, Urbana and Mattoon.
Because treatment is planned and completed at one location, patients traveling longer distances can often combine stages into fewer visits. Mention your travel distance when you call and we will schedule accordingly.
